Blinatumomab + inotuzumab sequential consolidation (R/R Ph-negative B-ALL, MRD-directed bridge to alloHSCT)

Deterministic view of the source YAML entity. Clinical authority remains with the cited source IDs and reviewer sign-off state.

Knowledge-base record for audit. Provisional clinical content: this is not a prescription. An oncologist must verify original sources, applicability and review status. Full rules and constraints are in the YAML.

Notes

Sequential immunotherapy strategy for adult R/R Ph-negative B-ALL using blinatumomab + inotuzumab as bridge to allogeneic HSCT — order tailored to disease burden and antigen expression. Rationale: TOWER + INO-VATE individually showed 40-80% CR rates; sequential application can rescue blinatumomab failures (CD19-loss escape) with inotuzumab and vice versa. Real-world series (MD Anderson Jabbour, others) show MRD-negative CR rates ~60-80% with bridge-to-HSCT in 50-70% of responders. Limit inotuzumab to ≤2 cycles pre-HSCT and avoid dual-alkylator conditioning (busulfan + melphalan + thiotepa) — VOD/SOS risk multiplies. STUB — pending TOWER ingestion + Co-Lead sign-off; sequencing details depend on local HSCT timing protocols.
