Notes

1L for high-risk APL (Sanz WBC >10) per ELN 2019. ATRA + ATO + idarubicin is preferred when ATO available; classic AIDA (ATRA + idarubicin alone) is acceptable alternative without ATO. Hyperleukocytosis-related DS prophylaxis and DIC management are mandatory. Cumulative anthracycline dose tracking. Pregnancy gating mandatory (all 3 agents teratogenic). STUB — requires clinical co-lead signoff before publication.
