7+3 + fractionated gemtuzumab ozogamicin (CD33+ AML, 1L; ALFA-0701)
Deterministic view of the source YAML entity. Clinical authority remains with the cited source IDs and reviewer sign-off state.
Knowledge-base record for audit. Provisional clinical content: this is not a prescription. An oncologist must verify original sources, applicability and review status. Full rules and constraints are in the YAML.

Notes
ALFA-0701 (Castaigne Lancet 2012): fractionated GO 3 mg/m² days 1, 4, 7 added to standard 7+3 in 1L de-novo AML age 50-70. 2-yr EFS 40.8% vs 17.1% (HR 0.58); 2-yr OS 53.2% vs 41.9% (HR 0.69). Hills meta-analysis 2014 (Lancet Oncol): GO addition reduces 5-yr mortality by ~20% in favorable-risk (HR 0.47) and intermediate-risk (HR 0.84); NO benefit in adverse-risk (HR 1.03). AML-19 (Burnett 2017) supports GO 6 mg/m² single dose in elderly unfit-for-7+3 but is a different regimen entity. Position: preferred 1L for fit CD33+ CBF / favorable / intermediate- risk AML where GO accessible. Daunorubicin dose 60 mg/m² (not 90) when adding GO due to additive cardiotoxicity. Hepatic VOD/SOS is the dominant safety concern — careful LFT monitoring + alloHCT timing (avoid GO within 90 days of planned alloHCT).
