Notes

STUB — pending Co-Lead sign-off. monarchE: 2-year fixed-duration adjuvant CDK4/6i for high-risk HR+/HER2- early BC. Key AEs: diarrhea (most common, usually G1-2, manageable with loperamide), neutropenia (CBC q2w x 2 months, then monthly), hepatotoxicity (LFT monitoring), VTE. Strong CYP3A4 inhibitors increase abemaciclib exposure significantly — dose reduction required. NO benefit beyond 2 years per current data; fixed-duration design. Concurrent endocrine therapy (AI or tamoxifen) is required throughout.
