Notes

Hypofractionated RT for GBM: two validated schedules: (1) 40 Gy / 15 fractions (2.67 Gy/fx, 3 weeks) — CCTG CE.6 schedule (2) 34 Gy / 10 fractions (3.4 Gy/fx, 2 weeks) — Nordic schedule Both are NCCN acceptable. 40 Gy/15 more commonly used when concurrent TMZ is added (better tolerability with 3-week course than 2-week). RT is not modelable per PROPOSAL §17 — this indication captures the TMZ chemo backbone; RT schedule is free-text MDT decision.
