MURANO trial (Seymour 2018; updated 2023): in r/r CLL after ≥1 prior line, VenR vs BR — superior 4-year PFS (57.3% vs 4.6%) AND superior 4-year OS (85% vs 67%, HR 0.41). Approved 2L+ for r/r CLL. 24-month fixed-duration with deep MRD-negative responses (~62% MRD-negative at end of treatment). Distinct from VenO (1L CLL14 schedule, 12 mo + obinutuzumab anti-CD20 instead of rituximab). TLS PROTOCOL CRITICAL: skipping ramp = fatal hyperkalemia + AKI.
