Notas

MASCC/ESMO 2016 + NCCN Antiemesis 2025 standard for high-emetogenic chemo (HEC = cisplatin ≥50 mg/m², AC, carboplatin AUC ≥4 (since 2017), cyclophosphamide ≥1500 mg/m², HD-cytarabine, others). Full 4-drug: (1) NK1 antagonist — aprepitant 125 mg PO day 1, 80 mg PO days 2-3 (or fosaprepitant 150 mg IV day 1 single dose; or NEPA combination netupitant-palonosetron 300/0.5 mg PO day 1 single); (2) 5HT3 antagonist — palonosetron 0.25 mg IV day 1 PREFERRED (single-dose covers acute + delayed) OR ondansetron 8 mg IV / 16 mg PO; (3) dexamethasone 12 mg day 1 with NK1 (or 20 mg without NK1), then 8 mg daily days 2-4; (4) olanzapine 10 mg PO QHS days 1-4 (PrOnD trial Navari 2017 NEJM — added to triplet, ~50% reduction in CINV failure). Olanzapine also substitutes for delayed-phase dexamethasone (steroid-sparing). For Ukraine where aprepitant unavailable, the best evidence-based alternative is ondansetron + dexamethasone + olanzapine triplet (NCCN supports this combination for HEC when NK1 unavailable). DRUG IDs missing in KB and to be authored: NK1 agents (aprepitant/fosaprepitant/rolapitant/netupitant), olanzapine, palonosetron, metoclopramide, lorazepam — flagged for PROD batch.
