Adjuvant pembrolizumab is the only systemic therapy with proven OS benefit post-nephrectomy in RCC (KEYNOTE-564 updated). Stage II high-risk (pT2G4), all stage III (pT3-4, N1), and M1-NED are the eligible groups. Stage I and low-risk stage II — observation only. Timing: must start ≤12 weeks post-surgery.
