Pediatric VAC + IE (Ewing-like) protocols are myelosuppressive and require strict hepatic / renal monitoring in infants (often <1 yr old). LVEF threshold 55% (not 50%) in pediatric oncology reflects long-life-expectancy sensitivity to anthracycline cardiotoxicity.
